New Patient Orientation
Welcome to Hauser-Ross Eye Institute! We appreciate the opportunity to serve you and look forward to seeing you at your upcoming visit!
In order to avoid delay in beginning your eye care when you arrive, you may print out these forms and fill out the information prior to your appointment. As a new patient, we ask that you please arrive 20 minutes before the start of your appointment. Please bring your insurance cards and driver's license or a photo ID.
As a service to you, we will be glad to file your insurance, however with today's health care restrictions, it may be worthwhile to take a moment to call the customer service phone number on the back of your card to be certain our physicians are part of your insurance plan.
New patient exams almost always include dilation of the eyes. Please be advised that this may affect your ability to drive safely so you may want to bring a driver with you. Please plan on spending approximately two hours with us for your first appointment.
